چکیده

Chronic recurrent multifocal osteomyelitis (CRMO) is a form of chronic nonbacterial osteitis (CNO) characterized by one or more lytic bone lesions with no identifiable cause1. The metaphyses of long bones and the clavicle are most frequently affected, although any bone may be involved. Most cases begin in childhood, and follow an intermittent course. Pain is the most common presenting symptom. Affected individuals are sometimes febrile, and acute-phase reactants may be increased, pointing to the inflammatory expression of this condition. Lesions may resolve at or before the time of puberty, but in some cases skeletal deformity and/or disability may occur2. We describe 2 children with CRMO whose symptoms persisted after trials of nonsteroidal antiinflammatory medication, glucocorticoids, and methotrexate (MTX). Combination therapy with MTX and etanercept resulted in amelioration of pain, accompanied in one patient by radio graphic improvement in a lytic bone lesion.

Bone scintigraphy in diagnosing chronic recurrent multifocal osteomyelitis

A 10-year-old boy was referred to us for evaluation of FUO accompanied with bone pain in both calves. Three hours after intravenous injection of 13 mCi of 99mTc-MDP, whole body scan in multiple spot views was performed. The scan showed symmetrical areas of diffusely increased tracer uptake in multiple long bones. Histopathologic evaluation confirmed osteosclrosis and fibrotic changes...

Chronic Recurrent Multifocal Osteomyelitis in a 9-year-old Boy

Chronic recurrent multifocal osteomyelitis (CRMO) is a rare aseptic, auto-inflammatory bone disorder. CRMO presents with bone pain with or without fever. The diagnosis of CRMO is a diagnosis of exclusion and should be included in the differential diagnosis of chronic inflammatory bone lesions in children. Cultures of the bone are typically sterile, antibiotic therapy does not result in clinic...

Chronic recurrent multifocal osteomyelitis of the spine and mandible: case report and review of the literature.

At initial presentation, chronic recurrent multifocal osteomyelitis may mimic acute hematogenous osteomyelitis; however, cultures of affected bone are sterile. Nuclear scintigraphy identifies additional foci of involvement that present concurrently or sequentially. Unlike acute bacterial osteomyelitis, chronic recurrent multifocal osteomyelitis seems unaffected by antibiotic therapy and typical...

Imaging of refractory chronic recurrent multifocal osteomyelitis succesfully treated with etanercept and methotrexate

Methods A 5-year-old boy presented with a 1 year history of right leg pain with limited range of motion and difficulty in walking due to flexion, abduction and external rotation of right hip attitude. No fever. Plain radiographs were normal. In the blood test, elevation of acute phase reactants was detected: ESR 41 mm/h, CRP 2.27 mg/dL. A Whole Body MRI (WB-MRI) scan revealed osteitic multifoca...
